WEED separate and apart from her husband, (she) does of her own free will and accord desire said adoption. It is ordered from this date Georgiana CLARK to be declared to be to all legal intents and purposes the child of said petitioners and that her name be changed from Georgiana CLARK to Alice Georgiana WEED.
